Le syndrome de Swyer est un hypogonadisme où un individu d'aspect féminin (c'est-à-dire de phénotype féminin) possède un caryotype avec des chromosomes sexuels de type XY mais où le gène SRY, responsable de la différenciation de l'embryon indifférencié en individu mâle à la sixième semaine de développement embryonnaire, est muté (c'est-à-dire a subi une mutation). Le gène SRY, qui d'habitude conduit à l'expression de gènes de différenciation des testicules (pas seulement mais notamment les testicules) ne s'exprimant pas, les gonades deviennent par défaut des ovaires qui ne synthétisent pas de testostérone. L'aspect est par défaut celui d'une femme mais le chromosome Y, bien qu'incomplet est présent.
